1. Introduction
This document outlines the Terms and Conditions (T&Cs) for the Collaborative Rewards Scheme (CRS), initiated by CW Vet Physio (referred to as "the Company"). The CRS aims to support collaborative relationships with veterinary surgeons by offering rewards for referrals leading to initial consultations, thereby enhancing veterinary physiotherapy service quality.
2. Eligibility
Participation in the CRS is available to veterinary surgeons licensed in the UK who refer clients to the Company, subject to acceptance of these T&Cs.
3. Referral Process
Referrals should be made to the Company using the veterinary referal scheme form. Eligibility for the CRS is limited to new clients paying an initial fee to the Company's services.
4. Reward Structure
The Company will pay a £25 (twenty-five pounds) reward for each referred client who completes an initial consultation. Rewards are calculated on a monthly basis based on completed consultations from referrals. The Company reserves the right to modify the reward amount with one month's notice.
5. Payment Terms
Payments are issued on the first of each month, with a reasonable margin to accommodate UK public holidays and weekends and the company’s specific hours of operation (including personal holidays). All payments will be made via bank transfer, with details to be mutually agreed upon between the Company and the veterinary surgeon.
6. Conditions for Void Payments
Rewards for referrals not leading to an initial consultation within two months are void. Rewards will not be issued for referrals resulting in cancelled or unattended consultations.
7. Record Keeping and Reporting
CW veterinary physiotherapy commits to maintaining accurate records of referrals and consultations. These records will be available at request of either party.
8. Data Protection and Privacy
All personal data exchanged will be handled in strict compliance with the General Data Protection Regulation (GDPR) and other relevant UK data protection laws, ensuring confidentiality and security of client information.
9. Quality Assurance
The Company commits to upholding the highest standards of veterinary physiotherapy services. Participation in the CRS must not compromise the quality of care, adhering to the standards set by the Royal College of Veterinary Surgeons (RCVS) and other relevant guidelines.
10. Termination Clauses
Either party may terminate their participation and should be communicated via email/phone.
11. Feedback and Continuous Improvement
Participants are encouraged to provide feedback on the CRS. The Company commits to utilising this feedback for continuous improvement of the scheme, fostering better collaboration and service quality.
12. Dispute Resolution
Disputes should be resolved through direct communication. Although active effort shall be made to resolve the dispute, final decisions on resolution are to be made solely by the company.
13. Amendments
The Company may amend these T&Cs at any time, with changes effective upon notification to participants.
14. Termination
The Company reserves the right to terminate the CRS or an individual's participation with reasonable notice.
15. Governing Law
These T&Cs are governed by the laws of England and Wales, ensuring alignment with local legislation and professional standards.
16. Acknowledgment
Participation in the CRS signifies acknowledgment and agreement to these T&Cs, including the commitment to confidentiality and the ethical framework guiding this initiative.
